Passport Services Resume with Temporary Adjustments to Procedures

Note: The City of Jacksonville has temporarily adjusted some Passport Service procedures following the COVID-19 event. The service is by appointment only. Face coverings will be required in order to meet with our Passport Agents until further notice. Building access will be limited to the Public.

Processing Status: The US Department of State says to expect delays in passport processing services. Unless you are a customer who needs a passport in the next 72 hours or for a life-or-death emergency, passport processing will likely be delayed. Learn more here.

Passport Services

Business or vacation taking you abroad? The City of Jacksonville is certified through the US Department of State as a Passport Acceptance Facility. We offer the following services:

  • Passport forms
  • Passport photos
  • Expedited and overnight services

Important documents checklist - The City will accept your completed application for a new passport and forward it to the Passport Services section of the U.S. Department of State for an issuance determination. All applicants submitting passport application Form DS 11 must:

  • Appear in person (Face coverings required to meet with Passport Agents, until further notice.)
  • Completed passport application (DS-11), typed or filled out in black ink
  • Proof of US citizenship (birth certificate, expired passport, naturalization certificate)
  • Submit proof of identity (driver’s license, military ID, other government issued ID, previously issued passport, etc)
  • Provide a recent 2 inches by 2inches high-quality photograph (Jacksonville offers photo service, see below)
  • Check or money order for City Services made out to the City of Jacksonville
  • Check or money order for USDOS Services made out to the US Department of State. See pricing information below
  • Please note that all documents must be original or certified copy
